A troubling pattern has emerged in the distribution of COVID-19 vaccines in the United States, with most vaccine sites located in white neighborhoods, making it challenging for Black and brown eligible residents to receive the vaccine. This issue, compounded by historical medical mistrust and ongoing systemic racism, is causing concern among health professionals.

Rachel Pearson, an assistant professor of pediatrics and medical humanities, finds the failed rollout of the vaccine to be intolerable. She believes the failure is due to the same systemic racism that puts Black, Latinx, Indigenous, and low-income communities at increased risk of air pollution.

The historical distrust toward government and medical systems arises from abuses like the Tuskegee Syphilis Study and ongoing medical racism, causing hesitation and lower vaccination rates in Black and Latino communities compared to White populations. Mistrust is further compounded by misinformation and inconsistent communication about vaccines, leaving people uncertain or unwilling to accept them.

Communities of color often experience poorer air quality, which worsens underlying health conditions that increase COVID-19 risks. Yet, these same communities may have less access to timely healthcare and emergency medical services due to historic underinvestment and structural racism. This confluence of environmental injustice and systemic racism leads to both higher COVID-19 exposure and severity among racialized minorities and barriers to vaccine acceptance and distribution.

In the U.S., the number of deaths from the coronavirus is approaching 464,000. The immigrant community in Little Village, a coronavirus hotspot in Chicago, is struggling to trust the government and seek the vaccine. Black, Native American, and Latinx people are dying at twice the rate of their white peers from the coronavirus. Communities of color are not receiving the COVID-19 vaccine at the same rate as their white peers.

Thousands of vaccine appointments remained unfilled in the Bronx by Friday, indicating a lack of trust among residents. In New York City, a mass vaccination center for the Bronx, which has the highest positivity rate in the city, was announced. However, in Chicago, the neighborhood of Little Village, a coronavirus hotspot, has received 20% fewer vaccinations than their wealthier neighbors.

Reneé Cross, senior director of the University of Houston Hobby School of Public Affairs, stated that a successful public immunization effort requires public trust. Building trust and ensuring equitable access to the COVID-19 vaccine is essential to protect vulnerable communities from the virus. An investigation by NPR found that access to the vaccine is not always easy, and this lack of access prevents anyone from standing a chance against the virus.

Medical and health professionals need to listen to communities of color to build trust and ensure this doesn't happen again in future disasters. Efforts to improve vaccine acceptance must address both trust-building through honest, consistent engagement and reduction of structural inequalities affecting these populations.
